As you begin your treatment with us, we would like to provide the following insurance information that will explain your health insurance coverage for your initial consultation and subsequent diagnostic procedures to determine the underlying cause of your infertility.

Most insurance carriers provide major medical benefits to diagnose the underlying cause of infertility, in which case the services typically covered are:

• Your initial consultation with Dr. Springer.



• Diagnostic testing and/or surgery to determine the underlying cause.

However, a few select insurance policies will not provide coverage for testing to diagnosis the underlying cause. In the event your insurance company does not offer coverage for a basic infertility evaluation, a cash package price is an option we offer to all our patients.

The above consultation and diagnostic charges will be submitted to your insurance carrier for payment. You will be responsible only for any applicable co-payments, co-insurance or deductibles.

At the conclusion of your diagnostic testing, you will meet with Dr. Springer to discuss the various treatment options available. At that time, you will be given a summary of insurance treatment coverage provided by your carrier, as well as our costs and payment policies. We believe that it is our responsibility to inform and empower our patients as to their benefits and rights in regards to their infertility insurance benefits.
